Patient Information - Billing and
Insurance
Advantage Physical Therapy accepts
private insurance, Labor and Industries, and
Medicare. We are also a preferred provider
with many insurance companies. We encourage
patients to call their insurance regarding
coverage and benefits prior to the initial
visit. Common requirements before coverage
begins include:
- Referral from a physician.
- Prior authorization before receiving
treatment.
- Co-pay and deductible obligations
required by the patient.
We file for services rendered directly
with the insurance company. Should insurance
payments be denied, payment is then the
patient’s responsibility. Co-pays and
deductibles are expected at the time of
service. We accept cash, check, Visa and
MasterCard.
