At Therapeutic Associates Advantage Physical Therapy it is especially important to us that you have a positive experience while you’re here. Therefore, we want to make sure you get your questions answered, provide you with current insurance and billing information, and give you the opportunity to complete our patient forms at your convenience prior to your visit.

In order to make better use of your time at our clinic, we encourage all new patients to come to their first visit 15 minutes early with their new patient forms completed. For your convenience, we offer two ways to complete these.

Frequently Asked Questions

What can I expect on my first visit?
Please arrive 20 minutes prior to your initial visit in order to complete the appropriate paperwork. Forms can be downloaded on this site to save time. Our office staff will be able to review your rehabilitation benefits quoted from your insurance provider. You will then be evaluated by a physical therapist to assess your impairments and current function. Based on this evaluation your treatment plan will then be discussed with you and you will also usually be started on a home exercise program. The initial treatment usually lasts 45 minutes.

What should I bring to my first visit?
Please bring your prescription from your physician and your insurance card. We can provide a gown or shorts, but you may bring your own appropriate clothing if you desire.

Who is involved in my rehabilitation?
Every effort is made to allow you to keep the same therapist through your rehabilitation. You may occasionally have some ancillary staff assist in your treatment. Your physician will be sent a copy of your initial evaluation, and progress notes will be periodically sent to your doctor to update him / her on your progress towards your stated goals.

Will my insurance cover the cost of treatment?
Advantage Physical Therapy accepts most insurance; however you are responsible to know your insurance benefits, since this can vary depending on the plan.

Billing and Insurance

Advantage Physical Therapy accepts private insurance, Labor and Industries, and Medicare. We are also a preferred provider with many insurance companies. We encourage patients to call their insurance regarding coverage and benefits prior to the initial visit. Common requirements before coverage begins include:

  • Referral from a physician.
  • Prior authorization before receiving treatment.
  • Co-pay and deductible obligations required by the patient.

We file for services rendered directly with the insurance company. Should insurance payments be denied, payment is then the patient’s responsibility. Co-pays and deductibles are expected at the time of service. We accept cash, check, Visa and MasterCard.
